An incident report is a formal written record of any noteworthy event that occurs at a childcare facility, including injuries, illnesses, behavioral incidents, medication errors, allergic reactions, or safety concerns. State licensing regulations require childcare providers to document incidents promptly and to notify parents or guardians, typically on the same day the incident occurs.
A thorough incident report includes the date, time, and location of the incident, the names of children and staff involved, a factual description of what happened, the response and first aid provided, the names of witnesses, and the signature of the person completing the report. Many states have specific forms or formats that must be used. Serious incidents, such as those requiring medical attention or involving suspected abuse or neglect, must also be reported to the licensing agency and, in some cases, to child protective services.
Maintaining accurate incident reports protects both children and providers. Reports create a record that can be reviewed during licensing inspections, help identify patterns that may indicate safety hazards, and provide documentation in the event of a legal claim.
