Incident reports document events like bumps, scrapes, bites, or behavioral incidents that occur during the day. Proper documentation protects children, informs families, and helps your center maintain compliance with licensing requirements.

Creating an Incident Report

  1. 1Go to the child's classroom or profile.
  2. 2Click Report Incident.
  3. 3Select the incident type (injury, illness, behavioral, other).
  4. 4Enter the date, time, and location within the center.
  5. 5Describe what happened, what action was taken, and any follow-up needed.
  6. 6Click Save. The report is logged and the child's guardians are notified.

Incident Report Requirements

  • Date and time of the incident.
  • Names of children involved.
  • Description of the event.
  • Action taken by staff.
  • Staff member who documented the report.
  • Guardian notification status.

Always complete incident reports on the same day the event occurs. Delayed reporting can create compliance issues during licensing inspections.
